Information you may choose to provide

Depending on the assessment path you use, this may include wellness goals, diet and lifestyle details, current supplements and doses, medications, health conditions, allergies or restrictions, laboratory information, physician notes, and documents you upload.

Why we use it

To prepare, review, explain, and communicate a personalized supplement recommendation.
To look for unnecessary ingredient overlap and possible safety concerns.
To determine whether a deeper review is more appropriate than an immediate recommendation.
If you order, to coordinate the selected formula, subscription, manufacturing, fulfillment, and customer support.

Service providers

VitOptics may share relevant information with service providers that help operate the assessment, ecommerce store, subscription, recommendation, communication, manufacturing, and fulfillment processes, subject to their contractual or legal obligations. We do not ask these providers to use your health and profile information for their own independent marketing.

Important healthcare privacy distinction

VitOptics is a wellness and supplement service and is not a healthcare provider. Information submitted through our assessment may not be protected by the Health Insurance Portability and Accountability Act (HIPAA). Please do not submit information or upload documents you do not want VitOptics and the service providers needed to deliver the requested service to process.

Your choice: You control which optional information and documents you submit. More context can support a more specific review, but you should not upload material you are not comfortable sharing for this purpose.

Questions, corrections, or deletion requests

Contact support@vitoptics.com to ask about, correct, or request deletion of information associated with your assessment, subject to information we must retain for legal, security, transaction, or operational purposes.
